DX: (1) Major depressive disorder, recurrent     (2) Anxiety/panic disorder      (3) Substance abuse (opiates) in remission. . . six years sober

Psych RX: Lexapro, 20 mg  & Klonopin (clonazepam)  2 mg daily as of April 22, 2013  (tapering off over six months after ten years... was on 4 mg daily as recently as Jan. 2013) 

Other RX:  Levothyroxine (for hypothyroidism)  112 mcg a.m.  , Prescribed diuretic for unexplained swelling in legs, Potassium supplement

Prior RX: Suboxone ("bupe") 2 mg 2x daily - ended July 2012, reinstated, ended for good January 2013.

Dx: Recurrent MDD, Dysthymia, OCD, Social Anxiety

Rx: Pristiq 50mg, Lamictal 400mg, Liothyronine (T3) 25mcg, Geodon 120mg

PRN:  Alprazolam and Ambien, OTC Benadryl for sleep
Non-psych: Bystolic 5mg (beta blocker for BP and anxiety), various Triptans (migraines), Propanolol 20mg (tremor)

Fail: Prozac, Wellbutrin, Lexapro, Celexa, Abilify, Zoloft, Hydroxyzine 25-50 mg, and Clonazepam .5 mg at bedtime
