Effexor and Ibuprofen

I know it's mentioned on the PI sheet to avoid Ibuprofen with Effexor and my pharmacist even goes out of her way to mention it. However, I get these blasted sinus headaches and menstrual cramps for which it's the only cure. I take it 2-3 times a week, only 1 extra strength tablet. I also take codeine phosphate daily for chronic back pain and it doesn't touch these things.

What's chemically happening that's wrong with this mix? VE? You around? I bet you know. Do you think it's a big deal?

That is a good question. How serious are the issues with ibuprofen and some psych meds? Tylenol doesn't work well for me. Ibuprofen works the best.

There shouldn't be many liver metabolic interactions, it's metabolized by one of the liver isoenzymes (2C9) that few psych meds inhibit or even use as a substrate. (Lockhart)

It is one of the NSAIDs that reduce clearance of lithium through the kidneys - major interaction risk there. IIRC, Naproxen is among the worst for that.

For ibuprofen and venlafaxine, NetDoctor states:

There may also be an increased risk of bleeding from the gut if ibuprofen is taken with the following medicines:

* anti-blood-clotting (anticoagulant) medicines such as warfarin

* antiplatelet medicines to reduce the risk of blood clots or 'thin the blood', eg low-dose aspirin, dipydridamole, clopidogrel

* SSRI antidepressants, eg fluoxetine, paroxetine, citalopram

* venlafaxine.

It's not guaranteed, but if you're susceptible to bleeding from taking ibuprofen, the combination just might get you.

It's not an issue for me and the meds I take (which do not include venlafaxine,) mainly because placebo works better for me than ibuprofen or naproxen, leaving me with acetaminophen as the only option (allergic to aspirin, myself.)

When I was taking Effexor, I had no idea there could be an adverse interaction between it and ibuprofen, and took ibuprofen as needed for headaches and such with no ill effects. That being said, I have no gut problems. If your stomach is sensitive to aspirin and nsaids, it would be worth looking into other pain killers.

I take ibuprofen with cymbalta all the time and have had no problems. Tylenol is useless as tits on a bull to me.

This the big thing with NSAID's(ibuprofen, naproxen, etc):

Do NOT EVER, EVER take them on an empty stomach. EVER!!!

At the very least take them with a glass of milk, but preferably with a meal.

That's my problem. I take lithium and ibuprofen at times. Like I said, tylenol doesn't work for me. I only take 600mg lithium.
