doghappy Posted May 2, 2009 Share Posted May 2, 2009 I take quite the cocktail. I have several problems but the one I'm trying to cure now is that around 4:00 p.m., my mood falls very noticeably. Things that did not bother me earlier in the day, bring me to the point of tears and I can get despondent. If I allow myself to make decisions from this point into the night, I regret them the next day. I can't think as clearly. In the morning, I take: Celexa, 80 mg Geodon, 40 mg Cytomel, 30 mg Provigil, 200 mg Adderall, 20 mg @1:00, I take: Provigil, 200 mg Adderall, 20 mg At night, I take: Lithium, 1200 mg Seroquel, 200 - 400 mg Topamax, 50 mg I'm wondering if the Celexa should be split up? sorrel Posted May 3, 2009 Share Posted May 3, 2009 It probably has little to do with the timing of doses (Celexa has a half life of 35hrs) and probably has more to do with the flavor of depression you have. Feeling worse at a particular time of day is common regardless of meds and their timing. Maybe you need a dose increase or med change rather than a dose timing change. gizmo Posted May 3, 2009 Share Posted May 3, 2009 I'm no expert, but if you are taking adderall twice a day, could the drop off you are feeling be the half-life of the adderall clearing from your system at around 4:00 pm? The only experience I have with stimulants is my son taking concerta, and it's longer-acting, so I could be blowing a bunch of smoke. I just mention it because you are looking for opinions. My current pdoc is a schmuck and won't even pay attention to the problem. That's an interesting thought about Adderall. I don't see how I can take it any later in the day as it keeps me up. Maybe I should try the extend release? It's so good to know it may be more the type of depression. I will be able to get a new pdoc in the next few months so maybe she can help me figure that out. booter26 Posted July 29, 2009 Share Posted July 29, 2009 I don't currently take any of the same meds as you (though I have at various times taken many of them), but I have a similar problem and after reading this, I'm thinking it's not just because of a need to have something distract me at all times. This is new to me and it sounds a lot like what I am experiencing. I take: AM: 200mg of lamictal 2mg xanax XR PM: 100 mg of lamictal and another 2mg of xanax xr and xanax IR as needed. Though my anxiety has improved greatly since increasing the lamictal from 200 to 300, around 4-5pm (and definitely by the time I get home from work) I am just...forlorn. I feel uncomfortable in my own skin and I cry easily, hyperventilate, etc. (panic attacks, interdespersed with plain old depression.) I can't stand being alone and I can't stand the thought of being with other people. I figured it was because I was distracted at work all day (though until recently, I was having 4-10 panic attacks a day at work over minor things...its really embarrassing to break down crying in front of co-workers, but fortunately, I work in an office full of men who have either crazy or pregnant wives, or in one case a crazy pregnant wife, so they're used to seeing women cry for no apparent reason), but I don't really know. At any rate, I have found that if I force myself out into the world or don't just go straight home to sit in my room and feel uncomfortable-- but go to exercise, be with a friend or my boyfriend, etc. I feel fine, or at least, distracted enough not to be freaking out. Medically speaking, for me.... Xanax IR doesn't seem to do the trick for the panic in the doses I'm prescribed and I'm trying to wean off of it (because it doesn't work, though I'm scared to death of the seemingly inevitable withdrawal symptoms) but the Xanax XR really evens me out. And since I'm from Southern California and could, theoretically, have a legal medicinal marijuana card (though I don't, but a lot of people from all walks of life around here do...a bipolar friend refers to it as her "medication of choice" ), I feel entitled to say, without recommending illegal drugs or self-medication, cannabis does seem to help a lot of *people I know* with anxiety and that afternoon/evening freakout. I don't know where you are, but any doctor located in the 13 states it's legal is technically authorized to issue a "letter of recommendation" and if you live in one of those states, based on the number of meds you are on, your pdoc may not be completely adverse to the idea. However, there is a liability issue that keeps many docs from doing so. Mine gave me the unofficial, "Does it help your anxiety? Does it help you sleep?" And when I said yes, he shrugged and said, "Whatever works." Good luck! I know this isn't super helpful, but if this website has taught me anything, it's that we are certainly not alone.
