vanderk Posted October 22, 2009 Share Posted October 22, 2009 Try lillycares.com. They do have a patient assistance program (though I have no clients on Cymbalta and can't vouch for the program). They should have a printable form that can be taken to the prescribing physician. Most PAP's have an easy first application. gizmo Posted October 22, 2009 Share Posted October 22, 2009 The lily PAP basically just asks you to send in your family's last tax year's tax returns to prove that your combined family income is less than program guidelines (which are listed on the website and vary depending on the number of people in your family). It's a fairly simple process with them, and you get the answer about 4 weeks. They ship four months of meds at a time to your doctor's office (the doctor has to write their script for you on the application form). If you're over 18 and file your own taxes you can put a household number as one and then only your income is taken into account.
