Post surgical Effexor concerns

I'm a newbie, and have some concerns about Effexor XR.  I have been taking 300 mg a day for about 6 years. I have anxiety and depression. It was originally prescribed by a psychiatrist, but my care has been moved to my gynecologist. Everything has been going fine, but about a year ago I found out I had a broken neck, had cervical spine surgery, and was on Norco off and on for about a year.  My surgery was in March, and I have been off the Norco for about a month.  Since that time, it seems like the Effexor isn't working at all.  I'm getting more depressed by the day, and I don't know why. Is it the Effexor, or the Norco withdrawal? I went off the Norco slowly under the supervision of the neursurgeon who did my cervical spine surgery. 

I have an appointment with my gynecologist, because I am 40 and found out I am going through, and have almost completed menopause.  I also have low thyroid.  I take, 1 mg of estradiol a day hormone replacement, and 75 mcg of levoxyl for my thyroid.

Needless to say I'm a pharmacological clusterfuck. I don't know what to do, and I have been noticing heart palpitations, and increased anxiety along with the downward spiral back into depression.  I'm starting to have difficulty functioning, so I'm very concerned.  If anyone has any experience with long term use of Effexor, with hormone and thyroid issues, please reply.

It's not unheard of for meds to quit working and need an increase after a while.  Actually, this almost always happens to me after about a year and a half.  Sometimes a dose increase helps and sometimes it doesn't. 

If possible, getting back under the care of a psychiatrist would be a good idea.
