Bueler
After 5 weeks, I would try increasing the dose. Any side effects?

hallowedink
You say that you feel a little better. Obviously, this is something that you need to bring up with your doctor, but my instinct would be to raise the dose and take it from there. 20mg is a low dose, so you have lots of wiggle room, and of course if it doesn't work there are plenty of other anti-depressants you can try. Are you in therapy? That would be the other thing that would help here. In my experience, therapy is the best place to deal with self-harm urges, because you can really get to the root of why you want to self-harm. Personally, I find CBT helpful for that, but obviously everyone is different. If you aren't in therapy, then I'd advise you to speak to your doctor and ask him to refer you to a therapist or point you to a counselling service. olga
This may be a particularly bad day for you, Nicola, but I wouldn't give up hope just yet. When I started with Cymbalta, it took almost 8-10 weeks before I felt the full effect. That isn't to say that you shouldn't contact your pdoc---if you feel that a higher dose is worth a try, by all means---call him/her. I think Hallowedlink makes some good points. A good therapist can work with you on the SI issue, and that might be more effective than expecting an antidepressant to do it all. Can you try some of the distraction techniques that are listed in the SI forum? Go for a walk, or listen to some music that's calming, or do a hobby like coloring mandalas or knitting? I'm glad you came here to talk about it. Depression really sucks. olga

jt07
I've got a doctors appointment on wednesday aswell as my counsellor so I'm going to ask for the dose to go up til then I've started taking 2 20mg prozac tabs instead of one, not sure if I shud have or not, so far its ok I'm glad that your pdoc and tdoc appointments are coming up soon. I'm not going to flame you for doubling your dose because I have done similar things in the past even though I don't recommend it. The problem is that any dose change in Prozac is likely to be felt after several weeks and not immediately. You need something that works sooner. Maybe you could talk to you pdoc about adding Abilify? Or maybe you could talk to you pdoc about adding something else to jump start the antidepressant such as a second AD that works faster. I take an SSRI + Remeron combo that works well for me. Remeron is generally believed to have a faster onset of action than Prozac. Anyway, I'm just throwing out ideas of what works for me so that you can discuss it with you pdoc. I hope you feel better soon.
