You are not required to disclose your disability: whether you do so or not is entirely up to you.  You can let your employer know that you have a disability and require accommodation if you need to do so.  You are entitled to 12 weeks of FMLA in a calendar year if you have worked 1,250 hours in the year prior to your leave.  You will likely have to provide a physician's documentation for your accommodation and there is a chance that your HR department could discover that your doctor is a psychiatrist, but your medical information must remain separate from your employee file and cannot be used to make unrelated decisions such as promotions, etc.  I hope this is useful to you.

If there are accommodations that can help, find them and ask for them.

I'm thinking about returning to work. I just got put down to once a week. But I'm kinda scared to. I think I can extend my leave a little longer, but I'm actually getting a little bored and antsy, so I'm not quite sure what to do.
