Some extended formulations can actually slowly and evenly release the medicine over 12 or 24 hours. Many can't do this reliably.  It depends on a lot of factors which often vary person to person. For the best shot at getting a steady drug level in your body multiple dose extended formulation meds have the best shot. You take 1350 mg daily as 3 doses of 450 mg. Sometimes extended release meds dump a lot right after you take them; they are not even in their dosing. So if you take 1350 mg as one dose you might get, say, 700 mg all at once. This can likely irritate your stomach. Hince 3 doses of 450 mg of the extended formulation.

They call it XR and you still have to take it three times a day? 

I don't know if mine is extended release at all but I take it twice per day, 12 hours apart. I suppose for TID you would be taking it once every 8 hours. Ask your pharmacist.

When I was on lithium, it was TID (300mg x 3). It sucked. I usually dosed 8-9a, 12-1p, 5-6p. I didn't do well if I dosed closer to bedtime. A lot of days I would end up BID because it wasn't feasible to stop and take a dose, especially mid-day.

Compliance with TID is awful anyway. For the most part I try to be consistent with my dosing, and I managed BID ok, but TID was not workable some days.
