Stressed about Disability

Although I am sloooowly recovering from major depressive episode followed by 2 hospital stays due to overdoses, my government sickness benefit is running out. They only grant you 15 weeks, after that they want you to apply for welfare. Anyways I am somewhat lucky as my place of work has disabilty insurance available after 17 weeks of absence. However to apply for this disability there is a 8 page application for me and a six page one for my Pdoc. Man, the questions on my form are quite cryptic and I am scared some of them are designed to fuck your claim up and give the insurance carrier ammunition to deny you. An example: What daily activities in your home are you unable to perform? I mean I shower, brush teeth, wipe own butt, comb hair, have sex,  I mean I'm mentally depressed, I can cook and feed myself. There isnt really anything physically I cant do. I may lay around in bed some days I dont know. Anyway, should I try and fudge ;) my answers a bit, Make things appear worse than they are throw in some hallucinations, voices in my head?  or be as honest as whatever. Has anybody else had expeirience filling these things out. If I was off due to surgery or shoulder replacement it would be easy. Describing a physciatric condition is  kind of grey area in terminology I think, and I'm  getting super stressed and hitting my Klonopin bottle more and more over this.  Thankx

First, try having a nice cup of tea to augment the klonopin's effects.  Look for something with "relaxation" or "bedtime" on the box.  I have a few favored blends, and they help. 

Second, you're right.  This is very much a grey area.  The forms were designed with physical limitations in mind.  The way I see it, they assume that if you can function at home, you should also be able to function at work and don't need their money.  What things do you normally do around the house that you don't or can't do right now?  Write those things down.  Perhaps your psychiatrist can help you by skimming over your application before you submit it.  My hospital has a social worker on staff who can sometimes help with things like this.  Having that little bit of extra human help might make things seem more managable for you. 

My forms for other purposes have weird things written on them, like "cannot operate manual can opener."  (I have both joint and mental issues.)  It looks goofy, but it's true, and the answers added up to enough to get me additional funding as a disabled student. 

DO NOT make things up.  Nine times out of ten, it will bite you in the ass.

It sounds like you are not in the United States.

But don't lie on the form!  In the United States, if you intentionally submit false statements in order to obtain benefits, you are guilty of a crime and are subject to imprisonment.

Also, I am sure they will check for inconsistencies.  You are applying on the basis of an affective disorder (major depression), not psychosis.  I would mention any problems with social functioning, leaving the house, appetite, lack of interest in activities.  Do you require assistance from others to do any activities (shopping, for instance?) Do others do gardening? Are there activities that you once did but now you refrain from doing because of your depression?  Any problems with concentration/persistence/pace of activities?  Be specific, for instance, how often do you lay in bed for most of the day, neglecting to do chores?  Does your anxiety cause problems, too?

Do your psychiatrist or psychologist's records back up these statements?

In the United States, it is difficult to get Social Security disability benefits for depression.  Many people with untreated depression work full-time daily.  Furthermore, many people are treated with medication to the point that they are capable of SOME full-time job that exists in the nation's economy.
