Controlled substances agreement form

Anyone else have to fill out a controlled substances agreement form for their clinic?

My pdoc read this form to me. It said I could not sell or abuse controlled meds (the klonopin and lunesta I'm on) or it could result in monitored medication management (already on that but it's for helping me stay on track with taking my meds) , or not getting prescribed any controlled substances, or having drug tests, etc. 

And it said if you are abusing or selling them you can be terminated from being seen at this clinic or made to have substance abuse treatment.

If you run out of your controlled substance early, it will not be filled unless it is extremely extenuating circumstances. 

They must be having trouble with people abusing and selling their meds. I've been going to that clinic for maybe 9-10 years now and never had to do this before.

With the current climate of people abusing and selling prescription drugs, I'm not surprised. That agreement doesn't seem to bad though. There is a member here who had to have a drug test every time she went to see her psychiatrist. I consider that over the line and I wouldn't agree to that. But the agreement you are talking about sounds reasonable. I'm sure that every one who is prescribed controlled substances at your clinic has to sign the same agreement. It's nothing personal.
