Not normal enough to keep a job, not crazy enough

I was denied for SSDI/SSI.  They say I didn't give any evidence as to why I couldn't work and I didn't give them any evidence past 9/30/04.  I was never asked for any "evidence."  I never filled out anything at all.  What gives?  And what do I do now?  I don't understand any of this stuff.

You write them a letter, typed, with the names and addresses (including zip codes) and phone numbers and fax number (if possible) of all of your doctors and clinics and hospitals (if applicable) that have treated you for your disabling condition.

Have you seen the website:

You also explain that you were not asked for any evidence to present, and that this should not be used as evidence to deny your claim.  You should also sign a copy of those authorization forms (see the above website for the official ssa.gov link) to print them out) so that they have permission for those sources to release medical records.
